Billing and Insurance

Teverbaugh, Croland and Mueller OG/GYN & Associates participates with most insurance companies in this area. This office holds that patients are ultimately responsible for the payment of their bills. You will be responsible for any services not covered under your contract or for services rejected by your insurance carrier. Patients are also responsible for any copays, coinsurances or deductible amounts. Please note that we do not accept Medicaid of any kind.

Deposits
If you do not have insurance or if a service is not covered by your insurance, we require a deposit prior to performing the service. Someone from our billing department will assist you in setting up a payment plan for the remaining balance due.

Co-Insurance / Co-Pay
If your insurance company requires that you will be responsible for part of a bill, this coinsurance must be paid prior to the services being rendered. Copays are to be paid at the time the service is rendered. It is required by your insurance carrier that we collect copays at that time.

Obstetrical Services
If you have no insurance, you will be asked to pay a deposit before your first visit with us. The balance of the total fees will be due in full before delivery. Please contact our billing department to arrange a payment plan.

Sterilization
Many insurance companies do not cover this surgical procedure. If your policy does not cover it, you will be asked to pay the entire fee prior to the surgery being scheduled.

Infertility
Patients will be required to check with their insurance company for benefit information before treatment for Infertility can begin. Many insurance companies require any testing, office visit and surgeries to be pre-certified. In addition, there may be specific requirements you need to meet before your insurance company will pay Infertility benefits. If your policy does not offer infertility benefits, you will need to speak with a member of our billing department to arrange payment for services prior to receiving them.

Miscellaneous

  • For your convenience, we accept Visa, MasterCard, Discover and debit cards.
  • Any overpayment by you or your insurance company will be refunded promptly
  • It is the patient’s responsibility to find out which provider, lab, hospital and / or surgery centers are covered by their insurance carrier prior to their visit.
  • Our panel is closed for all MEDICADE Plans
  • We accept Care Credit Financing
